Annual Wellness Visit
The Medicare Annual Wellness Visit (AWV) is a yearly appointment with one of our Primary Care Providers (PCP). At LA Medical, we are committed to providing patients with their own personalized prevention plan. Our mission with this plan is to identify possible health risks in order to help prevent illness from occurring as best we can.
Preventative Health Screening
At each of LA Medical’s Offices, patients have access to screening tests. Our Primary Care Providers (PCP) utilize these screening tests and provide in-depth physical examinations in order to identify potential illnesses as early as possible. Early identification of illness gives the opportunity for our PCPs in providing the appropriate treatment plan in order to help provide a better quality of life for all patients with illness.
Chronic Disease Management
LA Medical prioritizes a unified care approach to managing patients with Chronic Disease. In addition to standard routine check-ups, we make it a top priority to provide patients with the correct education, appropriate monitorization, and proper coordination of their illness in order to improve patients’ well-being as best we can.
Nutritional Management
At LA Medical, we also consider patient nutrition. Nutritional Management is such an important factor when it comes to prevention and treatment of illness. We truly wish to help educate and assist patients when it comes to maintaining proper diet and sufficient intake in vitamins. Our Primary Care Providers seek to work with patients on this aspect of medicine in order to ensure they are receiving a full-service treatment plan and lowering their risk of possible illness from occurring.
